The Adopter has agreed to adopt the above mentioned dog(S) according to the terms of this agreement:-
LCAR will remain the overseer of the dog(S) in any event, and the Adopter may not transfer or assign any of his/her rights to the dog(S) adopted under this agreement.
LCAR may terminate this agreement and repossess the dog(S) as described above at any time if it believes that the dog(S) have been maltreated or neglected in any way, or if the Adopter breaches this agreement. If the Adopter is unable or unwilling at any time to continue the adoption, he/she will notify LCAR and promptly return the dog(S) to LCAR by arrangement.
Adopter’s responsibilities
The dog(S) will immediately have the microchip registered into the Adopters name. (IN THE CASE OF A PUPPY OR KITTEN. WILL BE CHIPPED PASSPORTED AND RABIES VACINATED AT AROUND 3 MONTHS OF AGE AS IS LEGAL REQUIREMENT)
The Adopting person will be responsible for the care of the DOG(S) from the date of this agreement. This includes responsibility for arranging and paying for any veterinary treatment (including recommended inoculations, worming and flea treatment) which the(s) dog may require.
PUPPY ADOPTION…The adopting person also agrees that should the dog or bitch be ‘entire’, that they will take the responsibility to have him or her neutered or spayed at the earliest convenience at their own cost, (at least within 8 months of this adoption), and prove to LCAR that this has been completed with a note of verification from the vet.
The Adopter will promptly notify LCAR if his/her contact address or telephone number provided above changes.
The Adopting party may not use the dog(s) in any breeding programme whatsoever.
The Adopting party will allow a representative of LCAR to visit the dog(s) at any time on reasonable notice to check this dog’s welfare and well-being should LCAR deem fit to do so.
The Adopting party confirms that he/she is responsible for the dog(s) from the date of this agreement and acknowledges that LCAR has disclosed to the Adopter all relevant information which it has been given about the dog(s).
Note: That will include all information provided by the previous owner (where relevant), plus anything which LCAR has observed about the dog(s).
The Adopting party understands that they should be aware that basic rules apply when taking on an adopted dog – e.g. don’t let the dog off the lead for at least a couple of weeks or until you have established a command structure that the dog understands; also do NOT leave this dog unsupervised around small children/small animals until you yourself are confident of their welfare.
The Adopting party agrees that at no time can LCAR be held responsible for any damage or injury caused by the dog(s) from the date of this contract.
The Adopting party indemnifies LCAR against any costs, claims or liability arising as a result of the adoption of the dog(s) mentioned above upon the date of this agreement. Signature required below and a copy returned to last chance via Email.
LCAR confirms that if a qualified veterinary surgeon advises treatment for the dog(s), or that the dog(s) must be put down to prevent further suffering in the case of severe illness or injury, the Adopter may give any necessary permission to the veterinary surgeon on behalf of LCAR with a forwarding note of verification from the vet to LCAR.
